Dear OCS 2020 Attendee,

Please enter your name (as it will appear on your certificate), email address, and nursing license number, then rate each presentation session, the research posters as a group, and the Learning Resource Center (All questions are required to be completed before submitting).  

YOU CAN ONLY FILL OUT THIS EVALUATION ONCE.  ONCE YOU HAVE SUBMITTED A COMPLETED EVALUATION, YOUR CERTIFICATE WILL APPEAR ON THE SCREEN FOR PRINTING.
